Universal Credentialing DataSource is a single, national process that eliminates the need for multiple credentialing applications. Through this online service, providers complete one standardized application to meet the needs of all participating health plans and other healthcare organizations. It is available at no cost to providers.

Universal Credentialing DataSource was developed by the Council for Affordable Quality Healthcare (CAQH), a not-for-profit alliance of leading health plans and networks. The application can be accessed at www.caqh.org/credapp/
