Surrender of Third Party Administrator License
A third party administrator license does not expire in Louisiana. Once issued the license remains in effect unless surrendered, suspended or revoked. The annual report filing and fee is not a renewal filing and failure to file the report and the fee will not result in the expiration of the license but may need to other action.
To surrender a third party administrator license in Louisiana the licensee must provide written notice of the desire to do so and return the original license issued by the LDI or an affidavit stating that the original license has been lost. The notice must be signed by an officer of the licensee. The notice shall be submitted to:

A licensee must file an annual report as of the previous December 31 prior to surrendering the license.
